Position Summary:

This recruitment is for a Temporary Nurse Navigator (Clinical Nurse II) position through UC Irvine's internal temporary service. Position is hybrid, reporting three days on-site in Irvine or Orange. This position may require off-site visits, such as to a patient's home, and working during nights and weekends to assist patients.

Reporting to the Senior Nurse Navigator, the Temporary Nurse Navigator is responsible for the coordination and follow-up on implementation of a care plans for an assigned group of patients to ensure seamless care throughout the hospital, ambulatory practices, and varied and complex healthcare disciplines.

Duties include but are not limited to:

  • Serving as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care
  • Coordinating care with the physicians and members of the healthcare team
  • Providing access and coordination for a variety of patient groups

Compensation Range: $51.86/hr. or more, depending on experience

Required:

  • Previous experience working with physicians, nurses, and others to effectively develop and initiate patient care plans
  • Knowledge of medical terminology 
  • Ability to work evenings, weekends, holidays, and on-call on a rotational basis
  • Current BLS certification from the American Heart Association 
  • Valid CA Driver's License

Preferred:

  • Previous working experience and/or education and training in nursing
  • Experience utilizing EPIC EMR system
  • Experience utilizing Customer Relationship Managment platforms (CRM)
  • Demonstrated knowledge of various health insurance programs and government funded healthcare programs
