Position Summary: 

The Transitions of Care Pharmacy Technician, following established policies and procedures, provides technical support for pharmacists and clinicians to ensure safe and effective transitions of care at admission and discharge to support medication reconciliation and discharge pharmacy services. Collaborates with patient care team. The technician will employ a variety of strategies, appropriate for the patient/family /situation to obtain, update, and document a best possible medication history in the patient’s electronic health record. This includes, but is not limited to, utilizing electronic tools available (e.g. external prescription history and hospital electronic medical record system) and conduct patient interviews to enter the patient’s current medication list, and clarify allergy information into the electronic medical record. Technician will participate in the Pharmacy Department Performance Improvement projects/programs and collect data for workload measurements, as needed or required. The Pharmacy Technician may, under Pharmacist supervision, participate in education and training of students, interns, and residents.

Salary Range: 

 

Hourly Rate  Minimum   $28.61                   Midpoint          $32.10                   Maximum $35.58

 

Required Qualifications:

 

Ability to establish and maintain effective working relationships across the Health System

Ability to maintain a work pace appropriate to the workload

Excellent written and verbal communication skills in English

California Board of Pharmacy Technician Registration License or Pharmacy Intern Registration Certificate in good standing.

Minimum one year full-time relevant experience as a Pharmacy Technician

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Must exhibit verbal interview skills proficiency.

Excellent customer service skills and ability to navigate electronic medical systems and scheduling programs

Must demonstrate excellent customer service and communication skills

 

Preferred Qualifications: 

Minimum 2 years’ experience in outpatient/retail pharmacy and knowledge of medication therapy management or transitions of care

Knowledge of University and medical center organizations, policies, procedures and forms
