Position Summary: 

The incumbent is a patient educator-advocate who empowers patients to develop self-management skills with their health, which includes both preventative health and chronic disease management through improved communication, education and empathetic coaching. The goal of the community coach is to help patients and their families or significant others to cope with their health, manage symptoms and improve function and quality of life. Provides and/or supports health care services to individuals at UC Irvine sites or at other healthcare systems.

Salary Range: 

Hourly Rate  Minimum   $25.62                   Midpoint               $37.64                   Maximum $49.66

Required Qualifications:

Effective communication skills

Ability to establish and maintain effective working relationships across the Health System

Ability to maintain a work pace appropriate to the workload

Must demonstrate customer service skills appropriate to the job

Interest in the community served by UC Irvine Health (A Passion for Caring)

Bilingual skills in English and Vietnamese

Ability to read and communicate effectively in English

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Preferred Qualifications:

Bilingual skills in English and Spanish and/or Vietnamese

Knowledge of University and medical center organizations, policies, procedures and forms
